Context
Small producers, female headed households and PLWHA in rural
areas are vulnerable to a range of (e.g. food and waterborne)
diseases while having poor access to medical services. At the micro
level, poor referral systems and poverty lead to low use of health
services. Due to low antenatal service usage and stigma VCT
services are often not used. Abject poverty reduces local support
capacity for OVCs. At the meso level health
facilities are limited to the major urban areas in the districts,
and there is poor access to these centers. Availability of
antenatal care and PMTCT is limited. There is little influence of
the target groups on the performance of the health institutions,
while the motivation of health staff is low due to poor support and
working conditions. Coordination between different health actors
(government, NGOs, private sector) is lacking. Policies on harmful
traditional practices like FGM are poorly implemented. At macro
level there is a poor country strategy for
coordination of health activities. There is inadequate learning
within the health service on its performance and the incentive
package for health staff is not supporting their commitment.
Aim of the programme
The partners involved in the Health HIV
program aim to build local structures which can provide support to
health of vulnerable people (Home Based Care, Self Help Groups etc)
and spread information on preventive measures for health. These
structures will also be available to speak with health care
providers, like local government departments and clinics, to
promote the rights of the poor. Since most of the partners are
presently working on HIV prevention, care and support, the emphasis
of the program is on HIV affected groups, but it is expected that
other public health issues and disability issues will take a larger
share in the coming years as partners get exposed to more
activities. Policy influencing is difficult to work in due to
government legislation. Therefore it is a sensitive issue for the
partners and possibilities need to be carefully assessed.
